Essence Healthcare is a Medicare Advantage health maintenance organization with a Medicare contract. All Essence plans include Part D drug coverage. To enroll in an Essence plan, you must have both Medicare Parts A and B and reside in the Essence service area.

Please note that members must continue to pay their monthly Part B premium to Medicare, and that enrollment in an Essence plan is limited to specific times of the year. The benefit information provided herein is a brief summary, not a comprehensive description of benefits. For more information contact the plan. Benefits, formulary, pharmacy network, premium and/or co-payments or co-insurance may change on January 1, 2013. Contact us for more information.

Essence members must use network pharmacies to access their prescription drug benefit, except in non-routine circumstances, and quantity limitations and restrictions may apply. Members must also use plan providers except in emergency or urgent care situations. If you obtain routine care from an out-of-network provider without prior approval from Essence, neither Medicare nor Essence will be responsible for the costs.

You may be able to get Extra Help to pay for your prescription drug premiums and costs. To see if you quality for extra help, call 1-800-MEDICARE (1-800-633-4227). TTY users should call 1-877-486-2048, 24 hours a day/7 days a week. Or, you can call the Social Security Office at 1-800-772-1213 between 7 am and 7 pm, Monday through Friday. TTY users should call 1-800-325-0778. Or, call your State Medicaid Office.

The Medicare program has rated how well Essence performs in different categories. You may obtain this information by visiting www.medicare.gov or by calling us. Plan performance summary star ratings are assessed each year and may change from one year to the next.
